Your name, date of birth, email address and phone number are required to send an online consultation request. Other fields let you provide your National Health Index (NHI) number, referral source, insurance and GP details, the reason for your request, its urgency and additional information.

Where your request goes

The information you submit is sent by email to Dr Baecker's practice. The appointments team uses your request to arrange the next steps with you.
